My name is Ysabel. I just turned 40 years old. I was 38 when I lost my sight. It just happened so fast. I never thought that something like this would ever happen to me, but it did.

Ysabel suffers from diabetes and high blood pressure. When she failed to get proper treatment, the diseases caused her blindness and led to a series of near-fatal heart attacks, falls, and a stroke.

Maura (Ysabel’s Mother):
I don’t know much about the kind of sickness that she has. It is just tearing me apart to see her like that.

Barbara (Amerigroup Nurse Case Manager):
When I first meet Ysabel, back in March of this year she was in the hospital, she had two heart attacks, she had a stroke, she was going into kidney failure, and through all that she had never gone to her primary care physician. She was so overwhelmed with all of her medical issues that she did not know what to do.

Ysabel (Amerigroup Member):
I felt like giving up a lot of times. I just didn’t want to live.

Barbara (Amerigroup Nurse Case Manager):
She was having a very difficult time trusting people and believing that they would stay around to help her.

I just wanted to come over and check on how your medications are doing and if you’re having any problems. Going to Ysabel’s home made a huge difference in the fact that it wasn’t just a medical visit it was getting to know her and getting to know her surroundings and her family. Which is very important to her.

Ysabel tell me what you normally eat in a day.

Ysabel (Amerigroup Member):
I eat fruits, vegetables.

Barbara (Amerigroup Nurse Case Manager):
Is it okay if I check the refrigerator? Just to see what you have in there.

Ysabel (Amerigroup Member):
Yes mam.

Barbara (Amerigroup Nurse Case Manager):
I see you have your cranberry juice in there and your water. That’s great.

Ysabel (Amerigroup Member):
Yes mam.

Barbara (Amerigroup Nurse Case Manager):
…and vegetables and lots of fruits. That’s really good. That’s very very good.

Barbara (Amerigroup Nurse Case Manager):
Once we gained her trust then she started doing one thing at a time and before you knew it it made a huge difference. Just doing something simple as coordinating door to door transportation. It’s changed her life. She’s now getting dialysis, she is getting her medications, and she is able to go to her doctors appointments.

Doctor:
Let me listen to you Ysabel . Nice deep breath. Good. I was going to say are you taking all your medicines like you should be, but I guess you are because your blood pressure is wonderful.

Barbara (Amerigroup Nurse Case Manager):
I am so proud of her.

Doctor:
Me too.

Barbara (Amerigroup Nurse Case Manager):
She is working really hard.

Doctor:
Good. I will see you in dialysis.

Ysabel (Amerigroup Member):
Yes, mam.

Doctor:
Alright my dear. Good work. Thank you [Barbara], I know that you also work hard.

Maura (Ysabel’s Mother):
Barbara she is a wonderful person. Very wonderful person. I can’t ask for no... I just thank God that I found someone like her. I know someone to help me and my daughter.

Ysabel (Amerigroup Member):
I am just happy that I am getting a chance at life. I feel strong once again.
