NCQA Disease Management Seal 

Amerigroup's Disease Management (DM) Programs are designed to assist primary care practitioners and specialists in managing members with chronic diseases. Members are provided with care management and education by a team of highly qualified disease management professionals whose goal is to create a system of coordinated healthcare interventions and communications for the enrolled members.

As a part of Amerigroup's commitment to quality, we have earned Full NCQA Patient and Practitioner Oriented Accreditation for the following “Core” programs:

  • Asthma
  • Chronic obstructive pulmonary disease
  • Coronary artery disease
  • Congestive heart failure
  • Diabetes
  • Major depressive disorder
  • HIV/AIDS
  • Schizophrenia

NCQA DM Accreditation is a voluntary review process. Earning NCQA DM Accreditation is an indication that a DM program is dedicated to giving patients and/or practitioners the systems support, education and other assistance necessary to help ensure good outcomes and good care.

New Health Plan    

NCQA Health Plan Seal - New

NCQA Accreditation is an evaluation of how a new health plan ensures that its members are receiving high-quality care.  NCQA's New Health Plan Accreditation Program applies to health plans that are less than 36 months old. The program is distinct from NCQA's MCO Accreditation Program.

  • Amerigroup Community Care of Georgia has earned New Health Plan Accreditation.
  • Amerigroup Community Care of Ohio has earned New Health Plan Accreditation.
  • Amerigroup Community Care of Virginia has earned New Health Plan Accreditation.

Health Plan

NCQA Health Plan Commendable Seal 

NCQA Health Plan Accreditation evaluates how well a health plan manages all parts of its delivery system -- physicians, hospitals, other providers and administrative services -- in order to continuously improve the quality of care and services provided to its members. Health Plan Accreditation not only involves a rigorous review of a health plan’s consumer protection and quality improvement systems, but also requires health plans to submit audited data on key clinical and service measures (e.g., mammography screening rates; advising smokers to quit; consumer satisfaction).

  • Amerigroup Community Care of Tennessee has been awarded Commendable Accreditation Status for its Medicaid Products.
